Non-Brokerage Trustee Certification Form

Along with the Non-Brokerage Account Application, the Release of Information Page and the Change of Broker Dealer Form for any Trust Account, we need the Non-Brokerage Trustee Certification Form with the relevant sections for the Trust completed. Please ensure that ALL trustees sign and date the form. Remember that there are no POA's on a Trust account. if the trustees listed cannot sign, a copy of the trust will be required to see who is authorized to sign in that instance. We will also need a copy of the Title page of the Trust and the signature pages showing who the TTEE's are. We do accept the Certificate of Trust. 

Below is an explanation of each section on the Form. 

  • - State Law Governing Trust: This is the state where the Trust was signed. 



  • - Grantor: Also known as: Trustor, Donor, or Settlor. The Grantor is the person who created the Trust and who decides how the trust will operate 



      Type of Trust:
  • - Revocable Trust: The Trust can be changed or terminated until the Grantor dies.  

  • - Irrevocable Trust: The Trust cannot change once the Grantor signs. 

  • - Charitable Remainder Trust (CRT): Tax - Exempt Irrevocable Trust.

  • - Testamentary: A provision in a will that appoints the Trustee to manage the assets of the deceased.

  • - Nominee: The settlor appoints a nominee (trustee) to be the owner of the legal title to some property.



  • - Trustee: Individual or Firm that is required to manage and oversee the accounts. 


  • - Beneficial Owner: An individual who has a small percentage in the trust but enjoys the benefits of Ownership.
